PRE-ARRIVAL REQUIREMENTS

Complete the items below AT LEAST 90 DAYS PRIOR to beginning your clerkship.
The student will be required to complete a partial federal background initiated by Madigan GME. The GME office usarmy.jblm.medcom-mamc.mbx.gme@mail.mil will need the following information 90 days prior to a rotation.  Please put UW-Student Neurology in the subject line with each correspondence.

  • Clinic of rotation
  • Full mailing address
  • Phone number
  • Email
  • Date of birth
  • Social Security number
  • Place of birth
  • Letter of Good Standing (LOGS):  Required for this site.  Please go to your E*Value profile and download the LOGS to send.  Instructions if needed.

Once you submit this information, you will receive a a packet which MUST BE RETURNED NO LATER THAN 2 WEEKS AFTER YOU RECEIVE IT. The packet will contain:

  1. Information about whether fingerprinting is needed.
  2. A questionnaire which will need to be completed and signed by student.
  3. This packet will need to be returned with a copy of their school background done within the last two years.

If this packet is not received by GME 45 days prior to their rotation, the rotation will be cancelled. If students are returning to MAMC, this process will need to be done again.

TRAVEL & HOUSING

Mileage reimbursement: You will receive mileage reimbursement for one roundtrip to and from Madigan, and for your trips back and forth to Seattle for weekly class meetings. Contact Scott Bailey at sgbailey@uw.edu to arrange for reimbursement.
Housing: IHG Army Hotels Rainier Inn, 2107 Utah Ave JBLM, WA 98433. Check in any time the day before your clerkship begins. If you plan to check in on the first day of the clerkship, please contact the hotel so you do not lose your reservation. The hotel’s phone number is 253-964-0211. Ask to speak with the in-house reservationist.
Driving directions: Interstate 5 goes right through the post, Exit 120 will take you to the Fort Lewis main gate, which is open 24 hours a day. You will need to obtain a visitor’s pass from the main gate before checking into your lodging.


FIRST DAY & ORIENTATION

Arrive at Madigan on the first day of your clerkship at 7:30AM for in-processing. Check in with the GME office in the Medical Library on the 2nd floor of the Medical Mall (phone: 253-968-1511).
After processing with GME, go to the Neurology Service (which is also in the Medical Mall) and ask for Sharon Buckler (phone: 253-968-1399). She will introduce you to the attendings and residents on the service.
On Tuesday you will have a mandatory computer training session. Ms. Woodberry will provide you details for this session.
Equipment to bring: White coat and name tag, reflex hammer, tuning fork (128Hz), stethoscope, (Ophthalmoscope recommended).
Clerkship orientation: Because the clerkship orientation conflicts with in-processing at Madigan, you will need to do the clerkship orientation online. Go to the Didactics page and view the Seattle orientation video. You willl need to refer to your Pocket Syllabus (see below) when viewing the video.
Pocket syllabus: The pocket syllabus, referenced in the orientation video, will be available at Madigan. Please ask Sharon Buckler about it when you arrive. A printable (PDF) version is also available on the Pocket Syllabus page.

COURSE REQUIREMENTS

Didactics: Didactics take place on Thursdays, 1:00-4:00PM at the UW. The room schedule and details are on the Didactics page. Room information may change at the end of each rotation, so be sure to check the table the weekend before you begin.
Cases: Two case write-ups are due each week. The clerkship coordinator will email the assigned case numbers to you on or before the first day of the clerkship. Further information is available in the orientation video and on the Cases page. 
Patient log: You are required to log in the patients you have seen each week. The log is described in the clerkship orientation video. Additional details are on the Patient Log page.
Clinical & Bioethical CEX exercises: You are required to do both a clinical and a bioethical mini-CEX for this course. See the Mini-CEX page for details.
Presentations: Presentation requirements are described in the orientation video and are available on the Presentations page.
