Pre-arrival Paperwork Requirements
The application packet below must be completed AT LEAST THREE WEEKS PRIOR to beginning your clerkship. Fax or email all items in the packet to the HR Office at Virginia Mason.
- VIRGINIA MASON STUDENT PACKET
- VM will do their own background check. The 7-year address history IS needed.
- Page 3 of the packet is “Student Fitness for Duty Compliance Certification”. Dr. Kraus will act as the school representative and fill this out.
- Letter of Good Standing (LOGS): Required for this site. Please go to your E*Value profile and download the LOGS to send. Instructions if needed.

Course Information
Clerkship orientation: Orientation is at the UW at 8:00AM on the first day of your clerkship. The room number is on the Didactics page. Room information may change at the end of each rotation, so be sure to check the table the weekend before you begin.
After orientation: Report to Dr. Stoilova or Ms. Reisner at Virginia Mason Neuroscience Institute (address & map above).
Equipment to bring: White coat and name tag, reflex hammer, tuning fork (128Hz), stethoscope, (Ophthalmoscope recommended).
