Requirements Before Returning On-Site
Trainings:
All personnel must complete the COVID-19 Safety Training: Back to the Workplace before returning to work on-site at a University location.
Policies & Practices
- Maintaining social and physical distancing
- Practicing good hygiene
- Cleaning and disinfecting surfaces
- Using appropriate personal protective equipment (PPE), such as facemasks
COVID Prevention Plans
COVID-19 prevention plans will be provided for the central administrative offices in Neurology at UWMC and HMC.
It is a requirement that all research labs and clinical research activities have a specific COVID-19 prevention plan completed and submitted to Department Leadership.

Daily Attestation
A daily COVID-19 symptom attestation is required of all personnel working on-site to ensure the safety and public health of the UW campus and community. In accordance with University policy, please complete the appropriate attestation prior to each on-site work shift.
